Digital CBTe End User Terms and Patient Consent (UK)

These End User Terms apply to your use of Digital CBTe when you access it through an NHS Trust, clinic, or other care organization (“Care Organization”) that has invited you to use Digital CBTe.
1. Who we are
Digital CBTe is provided by Credo Therapies Ltd. (“Credo,” “we,” “us”). Your Care Organization makes Digital CBTe available to you as part of your care pathway.
2. Important safety information
- Digital CBTe is not an emergency service.
- If you are in immediate danger or need urgent help, call 999 (or your local emergency number) or go to A&E.
- If you are having thoughts of self-harm or suicide, seek urgent support from your Care Organization or local crisis services.
3. Your responsibilities
You agree to:
- Provide accurate information to the extent you choose to provide information.
- Keep your login details private.
- Use Digital CBTe for your own personal care and not on behalf of anyone else.
- Not misuse the Service (see Section 7).
4. Clinical relationship
Digital CBTe supports your care. It does not replace your clinician or Care Organization. Your clinician remains responsible for your care decisions and treatment plan.
5. Consent to use the Service and share information with your Care Organization
By clicking “I Agree” (or similar) you:
- Confirm you are at least 18 years old (or the age required by your Care Organization for this pathway). If you are under 18, do not use Digital CBTe unless your Care Organization has provided a specific pathway and consent process for minors.
- Agree to these End User Terms.
- Consent to Credo processing your information to provide Digital CBTe and sharing your information with your Care Organization, including your clinician team, as needed for your care and for operating the Service.
6. Your choices
Your Care Organization decides how Digital CBTe is used in your care. If you want to stop using Digital CBTe, you can stop at any time. You can also contact your Care Organization to discuss how your information is used in your care.
7. Misuse and restrictions
You must not:
- Try to access another person’s account.
- Upload viruses or harmful code.
- Attempt to reverse engineer the Service.
- Use the Service to harass, harm, or threaten others.
Credo may suspend your access if we reasonably believe there is misuse or a security risk. We will use reasonable efforts to coordinate with your Care Organization where appropriate.
8. Intellectual property
Digital CBTe and its content are owned by Credo or its licensors. You may use it only as allowed by these Terms.
9. Disclaimers
To the extent permitted by law:
- Digital CBTe is provided on an “as is” basis.
- We do not promise that Digital CBTe will be uninterrupted or error-free.
Nothing in these Terms limits liability that cannot be limited by law.
10. Changes
We may update these Terms from time to time. We will show the current version and effective date. Changes will not apply retroactively in a way that materially reduces your rights without notice.
